| Responsibilities: |
The Case Manager will assess, implement and evaluate the needs of patients for discharge planning and utilization review. They will review patient procedures and DRG status against criteria for Medicare, Medicaid, and other insurance coverage. Looking for a candidate with a strong clinical background and experience working with the aging community.

| Requirements: |
•2+ years in Hospital Case Management Position
•3+ years experience as a Staff RN in a Hospital Setting
•BSN- Bachelor of Science in Nursing
•RN -California Licensed Registered Nurse
•CCM- Certified Case Manager(Preferred)
•Computer Literate
•Strong Verbal and Written Communication Skills
•Capable Staff Leader with the ability to multi- task
